3. Complete each portion of the evaluation plan.
   1. Description of the problem—must be specific and must include the organization/healthcare entity problem,
   2. Description of the intervention—must be specific and must include where or which organization/healthcare entity this intervention will be implemented,
   3. Outcome measure section,
   4. Process measures section (note: you need three in this section)
   5. Balance measure section,
   6. Impact measure section,
   7. Communication plan—answer all four questions and be specific.
