Past Medical History (PMH):
Patient is Diabetic, controlled with Metformin 500 mg by mouth twice a day, Lantus 15 units SC bedtime. Hypertensive, controlled with Olmesartan 20 mg by mouth once a day. Atrial Fibrillation, controlled with Rivaroxaban 15 mg by mouth once a day and bisoprolol 10 mg by mouth once a day.

Endoscopic Ultrasound of the Pancreas. Solid mass in the head of pancreas 4 cms, infiltrating Wirsung duct. The solid mass impress to infiltrate the superior mesenteric vein. Perilesional node is detected, 1.5 cms, metastatic aspect. Fine needle aspiration (FNA) biopsy: Ductal adenocarcinoma.

Kaila Case Study

Kaila Case Study

 

The District of Columbia Early Intervention Program has referred Kaila to you for an evaluation. Kaila is 15 months old. She attends an Early Head Start program. Her mother, who is described as having some cognitive limitations, dropped out of high school at 15 years of age and now at 22 is attending a program for adults with developmental disabilities. Her mother is also homeless and lives in transitional housing. Kaila’s mother and her teacher at the Early Head Start program think an evaluation is appropriate.

Kaila Case Study

Although she consented to have Kaila evaluated, her mother is not very concerned about Kaila nor her development. She did express some frustration with Kaila’s “fussiness” and her lack of sleeping, however. Kaila’s primary child care provider is extremely concerned about Kaila’s physical growth. Kaila appears very small for her age of 15 months. Kaila’s mother is not sure how much Kaila weighs but did indicate that her pediatrician is following Kaila for her growth. Kaila is described as being a picky eater. She is bottle-fed and is difficult to feed. Although her mother indicated that she has no trouble feeding her at home, the primary child care provider indicated that Kaila often gags and throws up at child care. Kaila’s primary child care provider is also concerned about her temperament.

She describes Kaila as having a “flat” affect. Kaila often wants to be held and cries when other children approach her. Her development sounds as though it has been generally slow all around, although not likely 50% delayed. Kaila has just started to walk by herself. She babbles and says “ma ma”. She will wave “bye bye” as well. She will finger feed but will not use a spoon or drink from a cup. Her mother and social worker will accompany Kaila to the evaluation. Her primary child care provider will not be able to attend the scheduled evaluation.

Introduction to Your Assignment

Track your entire food and beverage intake (excluding supplements) for one day by writing down everything you eat and drink in an electronic format or in a notebook.

This is called a food journal, food diary, or food log.

Make sure to include the date you tracked your intake on your submission. The date must fall within the dates of this class.

Try to record your intake on a “normal” day for you. (Not a holiday, special occasion, or a traveling day unless your normal day includes traveling).

You will NOT be graded on what you eat and drink.

Instead, you will be graded on how well you documented the quantities and details of all the foods and beverages you consumed that day.

In this assignment, you will:

  • Journal your intake for an entire 24-hour period
  • Discuss factors that impacted your intake on this day. Connect these factors to the course textbook when possible.
  • Discover the fiber and added sugar content of some of the foods/beverages you are consuming

  • Details: What was the actual food or beverage item? Is there a brand name or a way to describe what the item is so that anyone reading your journal would know exactly which item you ate or drank?
    • Examples: 1 cup Kellogg’s bran flake cereal, ½ large red delicious apple, 3 Tbsp sugar free vanilla coffee creamer, etc.
  • Any extras. Did you dip your fries in ketchup (if so, how much?) Was there teriyaki sauce on the salmon salad? Did you take a few bites of dinner while you cooked?
    • Estimate all those bites and add them to your journal too!

STEP 4: Our resources this week shared that most people in the United States only get half the amount of fiber they need in a day. Using the three foods you selected above, find an alternative food that you could include in your diet that has more fiber than the items you originally included.
