Leading Healthcare Quality Improvement

Leading Organizations for Quality Improvement Initiatives

TOOLS FOR MEASURING QUALITY
How do we determine quality? Quality in other areas of our lives can be subjective, so as it relates to our nursing practice, how do we specifically ensure that quality is clearly defined and measurable?
Tools for measuring quality are used to assess the value measured, collected, or compared. These tools allow for subjectivity to be replaced with objectivity through data, formula, ranking, and analysis.
For this Assignment, you will explore at least three rate-based measures of quality. You will deconstruct each measure to explore your understanding of the it, including its importance and its impact on patient safety, the cost of healthcare, and the overall quality of healthcare.

THE ASSIGNMENT: (8–10 PAGES)
⦁ Describe the three rate-based measures of quality you selected and explain why.
⦁ Deconstruct each measure to include the following:
⦁ Describe the definition of the measure.
⦁ Explain the numerical description of how the measure is constructed (the numerator/denominator measure counts, the formula used to construct the rate, etc.).
⦁ Explain how the data for this measure are collected.
⦁ Describe how the measurement is compared externally to other like settings and differentiate between the actual rate and a percentile ranking. Be specific.
⦁ Explain whether the measure is risk adjusted or not. If so, explain briefly how this is accomplished.
⦁ Describe how goals might be set for each measure in an aggressive organization, which is seeking to excel in the marketplace. Be specific and provide examples.
⦁ Describe the importance of each measure to a chosen clinical organization and setting.
⦁ Using the websites and resources you can choose a hospital, a nursing home, a home health agency, a dialysis center, a health plan, an outpatient clinic, or private office. A total population of patient types is also acceptable, but please be specific as to the setting. That is, if you are interested in patients with chronic illness across the continuum of care, you might home in a particular health plan, a multispecialty practice setting or a healthcare organization with both inpatient and outpatient/clinic settings.
⦁ Note: Faculty appointments and academic settings are not permitted for this exercise. For all other settings, consult the instructor for guidance. You do not need actual data from a given organization to complete this Assignment.
⦁ Explain how each measure you selected relates to patient safety, to the cost of poor quality, and to the overall cost of healthcare delivery. Be specific and provide examples.

Healthcare Quality Improvement Project

Leading Organizations for Quality Improvement Initiatives
APPLYING PROJECT MANAGEMENT APPROACHES FOR A QUALITY IMPROVEMENT PRACTICE GAP
Project management allows for a clear and focused workflow to approach an issue or task. These approaches streamline a process, allow for checks and balances, and ensure all stakeholders are active participants in the process. Project management approaches often utilize SMART objectives to define and set the objectives for the project.
Each letter of the acronym SMART defines a different criterion for the objective. A SMART objective is specific, measurable, assignable, realistic, and timely. These objectives help to steer the work and direct the stakeholders to the completion of the project.
For this Assignment, you will reflect on project management approaches that could be used to address a quality improvement practice gap. You will develop SMART objectives for the planning and execution of a quality improvement project and will consider potential project management approaches or activities that could be used in executing this project.

Nursing Evidence-Based Practice Research

Research for Evidence Based Practice

QUANTITATIVE METHODS: ANALYZING QUANTITATIVE DATA
INTRODUCTION

Research allows us to learn more. It allows us to go beyond simple questioning and curiosity. It provides us the opportunity to dig deeper, search for outcomes, and explore meaning. Throughout your practice, you will experience the need for research. Whether it is in your educational journey or within your career, research is an integral skill set for your role as a DNP-prepared nurse.
Research begins as a question which leads to the study and collection of data. Through understanding and examining variables, determining levels of measurement, and analyzing outcomes, the question presented comes into clearer focus. This week, you consider the role of research in the field of nursing. You will explore variables in research and analyze data to develop understanding and meaning.
⦁ Analyze independent and dependent variables for research questions.
⦁ Identify levels of measurement for independent and dependent variables
⦁ Analyze approaches for addressing advantages and challenges in the data analysis for variables and levels of measurement.
⦁ Evaluate frequency and descriptive statistics.
⦁ Summarize descriptive statistics.

Nursing Evidence-Based Practice Research

Nursing Evidence-Based Practice Research

DISCUSSION: LEVELS OF MEASUREMENT
What is the incidence of blood clots from COVID-19 in females over the age of 35?
The above question is an example of a research question. A research question consists of three parts and guides the methods and approaches in which you will study the question to find answers. The research question includes: the question, the topic, and the population or variables. In the example provided above, the question is examining the prevalence of blood clots from severe COVID-19 in a selected population. From this question, the variables can be assessed, considerations can be analyzed, and populations can be sampled in order to guide the research.
During Week 2, you developed a research problem statement based on a topic of interest to you or your specific area of practice. Using this research problem statement, you will develop a research question. “A research question is a concise, interrogative statement that is worded in the present tense and includes one or more of a study’s principal concepts or variables” (Gray & Grove, 2020). These questions typically point to the type of study that will be conducted and serves as a guide for the research.
For this Discussion, reflect on your research problem statement. Consider the independent and dependent variables of your research problem through the construction of a research question. Reflect on the potential levels of measurement for your variables and the rationale for the labels, as well as consider the advantages and challenges that you might experience in the statistical analysis of your proposed variables.

Nursing Theory Discussion

Nursing Theory Discussion

WEEK 1 _

Nursing Theory Nursing theory plays a crucial role in both nursing education and practice. Its integration can vary depending on the specific nursing program and healthcare institution. Here is how nursing theory is typically incorporated: Nursing Education

1. Nursing programs, whether at the undergraduate or graduate level, often include courses in nursing theory (Younas & Quennell, 2019). These courses provide students with a foundation in various nursing theories and their application to patient care.
2. Nursing students learn about different nursing theorists, such as Florence Nightingale, Jean Watson, and Dorothea Orem, and explore how their theories can be applied in real-world healthcare settings.
3. The integration of nursing theory into education helps students develop a strong theoretical framework for nursing practice (Younas & Quennell, 2019).
Nursing Practice

Nursing Theory Discussion

1. In clinical practice, nurses often use nursing theory as a guide to provide patient-centered care. For example, theories like the nursing process (assessment, diagnosis, planning, implementation, and evaluation) are used to plan and deliver care.
2. Specific theories, such as those related to geriatric and cardiac nursing, may be applied to understand and address the unique needs of your primary client population. For example, theories on aging and chronic illness management can inform your practice (Younas & Quennell, 2019).
3. Nursing theory can help nurses in decision-making, critical thinking, and problem-solving in complex patient care situations.
In my case, as a nurse with 12 years of experience working with a geriatric and cardiac population and serving as a case manager you likely integrate various nursing theories into your practice to provide comprehensive care. You may use theories related to gerontology and cardiac care to assess, plan, and deliver care that meets the specific needs of your patients. The integration of nursing theory into practice can improve patient outcomes, promote evidence-based care, and enhance the overall quality of nursing care (Younas & Quennell, 2019). It also provides a common language and framework for communication and collaboration among healthcare professionals. Use APA referencing style.

Unit Plan and Commentary

This assignment parallels Depta Task 1: Planning for Instruction and Assessment. For this
assessment, you will first plan 3–5 consecutive mathematics lessons. For the purpose of this
assignment, we will describe these lessons as a unit (this may or may not coincide with a “unit” in
your textbook or curriculum map). Your unit should be consistent with the Common Core State
Standards for Mathematics and the Principles to Actions (NCTM, 2014). The unit should reflect a
balanced approach to mathematics. This means your unit should include lessons in which.
students have opportunities to develop:
• conceptual understanding
• procedural fluency
• mathematical reasoning and/or problem-solving skills
• precise communication
In addition to the actual lesson plans, you will provide justification for your plans in the
Commentary section.

Unit Plan and Commentary
For this assignment, you will complete and submit:
1. Contextual Information about your class (maximum: 3 pages) – OPTIONAL
For those of you who are currently teaching, it is recommended that you complete the
Contextual Information so that you are familiar with this component of the Depta before
you actually have to submit it. However, since not everyone is currently teaching, this is an
optional element of the assignment.
2. Lesson Plans & Instructional Materials (handouts, PowerPoints, etc.)
You are required to submit 3-5 lesson plans AND any instructional materials to be used in
the lessons. NOTE: These lesson plans are NOT the abbreviated versions that you are able
to submit in your teaching setting. The Depta lesson plans must be detailed enough for
the reviewer to understand exactly what will happen in the lesson from beginning to end.
For this reason, it is necessary to include any instructional materials that will be used in
the lesson. Failure to include the associated materials (either by uploading to Canvas or
providing a link) will result in a grade deduction.
3. Commentary
The Depta Commentary is where you provide explanation/justification for your lesson.
plans. For this class, we are doing a “practice” version of the commentary so that you are.
familiar with the prompts and the process. However, the more detail and examples you
include in this practice run, the better prepared you will be for the actual Depta.

OPTIONAL Context for Learning
For the purpose of this assignment, the Context for Learning is OPTIONAL. However, IF YOU ARE
TEACHING, I strongly encourage you to submit the contextual information for your class. This is a
required component of the Depta. For those of you who are doing the Depta next semester, it is.
especially important to get some experience not only writing up the Context for Learning but also
aligning your Commentary response with this information.
Your School
1. In what type of school do you teach? (Elem, Middle, High)
2. Is it urban, suburban, or rural?
3. List any special features of your school or classroom setting (e.g., charter, coteaching, themed magnet, remedial course, honors course) that will affect your
teaching in this learning segment.
4. Describe any district or school requirements or expectations that might affect your
planning or delivery of instruction, such as required curricula, pacing plan, use of
specific instructional strategies, or standardized tests.
Your Class
1. What is the name of the course (e.g., Algebra II) or the grade-level of the class?
2. What is the class schedule (e.g., 50 minutes every day, 90 minutes every other day)?
3. Is there any ability grouping or tracking in mathematics? If so, please describe how it.
affects your class.
4. Identify any textbook or instructional program you primarily use for mathematics
instruction. If a textbook, please provide the title, publisher, and date of publication.
5. List other resources (e.g., electronic white board, graphing calculators, online
resources) you use for mathematics instruction in this class.
6. Number of students:
Male: Female:
7. How many students in your class have IEPs or 504s? Describe supports,
accommodations, or modifications for these students.
8. How many students in your class are English Language Learners? Describe supports,
accommodations, or modifications for these students.
9. How many students in your class are struggling readers? Describe supports,
accommodations, or modifications for these students.
10. How many students in your class are underperforming or have gaps in academic
knowledge? Describe supports, accommodations, or modifications for these students.
11. How many students in your class are gifted? Describe supports, accommodations, or
modifications for these students.

LESSON PLANS
Write a lesson plan for each lesson in the unit. Your lesson plans should be detailed.
enough that a substitute or other teacher could understand them well enough to use.
them.
Your lesson plans should include the following information:
• State Standards and Common Core Standards (if you are using the TN standards,
you only need to list the state standards)
• Learning objectives associated with the content standards. What are the specific?
objectives students will meet in the lesson.
• Informal and formal assessments used to monitor student learning, including
type(s) of assessment and what is being assessed
• Instructional strategies and learning tasks (including what you and the students
will be doing) that support diverse student needs.
• Questions that you will ask the students
• Instructional resources and materials used to engage students in learning (submit
all accompanying materials that you would use in this lesson, including student.
assessments)
Individual lesson plans must be no more than 4 pages in length. You will need to condense.
or excerpt lesson plans longer than 4 pages.
For the purpose of ICL 7993 and ICL 4914, you will be required to use the Ready2Teach.
Lesson Plan template. While I recognize that those of you who are currently teaching are
typically, not required to create such detailed plans, the goal is to prepare you for success.
on the Depta. For this reason, we will use the Ready2Teach template (available in the
Course Materials module) for this assignment.

a. Explain how your understanding of your students’ prior academic.
learning, personal/cultural/community assets, and mathematical
dispositions (from prompts 2a–c above) guided your choice or
adaptation of learning tasks and materials.
b. Describe and justify why your instructional strategies and planned supports.
are appropriate for the whole class, individuals, and/or groups of students.
with specific learning needs. Be sure to address how your strategies and
supports meet the needs of students with IEPs, English language learners,
struggling readers, underperforming students or those with gaps in
academic knowledge, and/or gifted students.
c. Describe common mathematical preconceptions, errors, or misunderstandings within your central focus and how you will address them.

4. Supporting Mathematics Development Through Language
a. Language Function. Identify one language function essential for students to
learn the mathematics within your central focus. Some examples of
language functions include compare/contract, conjecture, describe,
explain, and prove.
b. Identify a key learning task from your plans that provides students with opportunities to practice using the language function identified above.
Identify the lesson in which the learning task occurs. (Give lesson
day/number.)
c. Additional Language Demands. Given the language function and learning task.
identified above, describe the following associated language demands (written
or oral) students need to understand and/or use:
 Vocabulary and/or symbols
 Mathematical precision (e.g., using clear definitions, labeling axes,
specifying units of measure, stating meaning of symbols), appropriate to
your students’ mathematical and language development
 Syntax
 Discourse

Unit Plan and Commentary

d. Language Supports. Describe the instructional supports (during and/or
prior to the learning task) that help students understand and successfully
use the language function and additional language demands identified in
prompts 4a-c. In other words, how are you providing support for students to
meet these academic language demands?
5. Monitoring Student Learning
In response to the prompts below, refer to the assessments you will submit as
part of the materials for your unit.
a. Describe how your planned formal and informal assessments will provide direct evidence of students’ conceptual understanding, procedural fluency,
and mathematical reasoning and/or problem-solving skills throughout the
unit.
b. Explain how the design or adaptation of your planned assessments allows students with specific needs to demonstrate their learning. In
other words, how will your assessments allow the following students.
to demonstrate their learning: student with IEPs, ELL students,
struggling readers, underperforming students or students with gaps in
academic knowledge?
more detail and examples you
include in this practice run, the better prepared you will be for the actual Depta. Use APA referencing style.

Education Parent Training Plan

Parent Training Plan Assignment Instructions
Overview

Individuals with ASD are uniquely impacted by sensory needs at home and school. Both hypo and hypersensitivities can impact each of an individual’s senses and impact communication, behavior, and social responses. As educators, we must support families in their awareness of how these sensory needs manifest and provide support recommendations. You will review the current literature, on sensory processing disorders related to tactile, vestibular, proprioceptive, visual, and auditory functions to develop strategies that parents can use with their children at home.

Education Parent Training Plan

Instructions

Create a family-friendly training plan to address the sensory impacts of students with ASD. Utilizing the Jerry Falwell Online Library research each sensory area and compose a final paper that includes the following elements:
A detailed description of each sensory/motor processing disorder highlighting how parents can recognize when their child may need a professional evaluation and how parents should best document their concerns.
A of five family-friendly activities to support the development of each sensory disorder, for a total of 25 activity recommendations. Submit these activities as a bulleted list with a brief description.
For each of the sensory/motor disorders, develop three strategies to address and overcome challenges parents might face when a child in the home has the disorder, for a total of 15 recommended strategies. Submit these strategies as a bulleted list with a brief description.
Recommendations on how the parents can keep data on progress to determine intervention/strategy success.
Include a personal reflection response to your parent training. Discuss your personal opinion and beliefs about this parent training, what you foresee as being challenging for parents implementing this plan, and what you might do to further support parents to find success with your plan.

Education Parent Training Plan

The final assignment should:
Be 5-8 pages, double spaced, including reflection but excluding title page and reference page.
Follow all current APA formatting guidelines.
Cite a minimum of five sources.
Two sources must include the course text and the Bible.

Positive Learning Environment Plan

Establishing A Positive Learning Environment Plan
Instructions: Develop a plan for the management of the classroom based on
educational research. The plan should include effective classroom management to
include classroom flow, time management (e.g., transition), and a range of strategies to
promote positive relationships and cooperation in the classroom.
1. Personal Theory: 1 page
a. Style/Philosophy, i.e., culturally relevant pedagogy. Or one of the five
orientations to education, such as progressivism or social reconstructionist
b. What is your purpose for teaching and how does your teaching style.
(inquiry-based?) inform your pedagogy?
c. What age group or grade is this classroom management plan designed.
for?

4. Rules, Routines, and Procedures-Classroom Rules & Procedures
Rules: This section should provide a compelling narrative that classroom
procedures are well-planned and consistent with the routines and times of the
instructional day. In your narrative, discuss the following.
a. Out-of-room policies
b. Materials and Equipment
c. Seatwork and Instruction Procedures
d. Managing Cooperative Groups
■ Group arrangement with seating arrangement
■ Example: Students will be arranged in four ability groups and will
