Disorderly Conduct

 

The sound of Matt and Peter’s arguing is familiar to everyone in the office by now. In an effort to make the best use of space and ensure a free flow of discussion and ideas, the founder of Markay Design had decided to convert the one-floor office of the company to an open plan with no walls between workers. The goal of such a layout is to eliminate boundaries and enhance creativity. But for Matt and Peter, the new arrangement creates a growing sense of tension as they share a large desk.

 

The argument boils down to the question of workspace order and organization. Peter prefers to keep his area completely clean and clear, and he keeps a stack of cleaning wipes in a drawer to eliminate any dust or dirt. Matt, on the other hand, likes to keep all his work visible on his part of the desk, so sketches, plans, magazines, and photos are scattered everywhere, alongside boxes of crackers and coffee cups. Peter finds it hard to concentrate when he sees Matt’s piles of materials everywhere, while Matt feels he can be more creative and free flowing when he is not forced to clean and organize constantly. Many of Matt and Peter’s co-workers wish they would just let the issue drop. The men enjoyed a good working relationship in the past, with Peter’s attention to detail and thorough planning serving to rein in some of Matt’s wild inspirations. But of late, their collaborations have been derailed in disputes.

Everyone knows it’s not productive to engage in conflicts over every small irritant in the workplace. However, completely avoiding conflict can be equally negative. An emerging body of research has examined “conflict cultures” in organizations. The findings suggest having a culture that actively avoids and suppresses conflicts is associated with lower levels of creativity.

 

Moreover, cultures that push conflict underground but fail in reducing the underlying tensions can become passive-aggressive, marked by underhanded behaviour against other co- workers.

 

Ultimately, finding a way through the clutter dispute is probably going to be an ongoing process to find a balance between perspectives. Both Matt and Peter worry that if they cannot find a solution, their usually positive work relationship will be too contentious to bear, and that would be a real mess

In this Assignment, you will use the following patient examples to write a 5- to 6-page paper on considerations you have for how you might de-prescribe. Patient Examples:

Patient 1: A 36-year-old male presents to your office being prescribed by his primary care physician (PCP) for the past 3 years an opioid analgesic medication for a work accident. He has chronic pain and is attending a pain clinic. It is determined the best course of treatment for pain is to remain on opioid medication. The patient is also being prescribed clonazepam 1mg BID for “relaxation” and panic attacks.

Patient 2: A 42-year-old female on alprazolam 1mg BID for panic attacks. Panic attacks have been in remission and the patient wants to taper off the medication. But, every time she has attempted to do so in the past, she experienced withdrawal effects. She is wondering how to safely taper off the benzodiazepine medication without having withdrawal effects.

Patient 3: A 24-year-old female prescribed lorazepam 1mg TID for generalized anxiety disorder. She recently found out she is pregnant (9 weeks gestation). She was referred to you by her OB-GYN to discuss this medication for her current situation. The patient is wondering if she can stay on the lorazepam through her pregnancy and postpartum, as it is an effective medication for anxiety symptoms. She plans to exclusively breastfeed for the first 6 months postpartum. She has not had any other trials of medication to treat anxiety as lorazepam has been effective.

Patient 4: A 71-year-old-male who comes to see you at the insistence of his daughter. His daughter expresses concern of memory loss and is wondering if he has the beginning stages of dementia. He is forgetful and seems to be tripping on things or walking into walls, although he has lived in the same home for the past 35 years. The patient does not agree with his daughter but does admit he has had a “few stumbles and falls” lately. Medication reconciliation shows the following medications: metoprolol ER 50mg q day, omeprazole 20mg q day, clonazepam 1mg TID, levothyroxine 75mcg q am. His daughter is wondering if he should be started on a “dementia medication.”

 

ASSIGNMENT

Answer the following questions using the patient examples described above.

Patient 1

  • What are the concerns of the patient remaining on the opioid medication and clonazepam?
  • How might you educate the patient about these risks and concerns?

The patient agrees that he should not continue both medications in combination. He would like to “get off” the clonazepam but worries about “bad withdrawals” that he’s heard about from stopping clonazepam “cold turkey” and is concerned about re-occurring panic attacks. How might you respond to the following:

  • How would you instruct the patient to taper off clonazepam?
  • What other medication would you recommend for the patient for the treatment of his panic attacks? Keep in mind, he will continue the opioid medication for pain relief.
  • How would you start the new recommended psychotropic medication for the patient?
  • Discuss one legal, ethical, or social consideration with the treatment plan.

Patient 2

  • The patient reports withdrawal symptoms when previously tapering off the alprazolam. What symptoms are common withdrawal symptoms from this medication?
  • Provide the patient education of withdrawal symptoms that range from common and less serious to withdrawal symptoms that are a cause for concern and that should prompt patient should seek medical attention.
  • Given the patient’s history of having withdrawal effects from attempting to taper off alprazolam, what longer-acting benzodiazepine would you choose to convert the patient to?
  • What is the dose you would prescribe and how would you taper off the medication?

Patient 3

  • Review the potential risks, benefits, and side effects of continuing lorazepam throughout the pregnancy and postpartum for both the patient and fetus.
  • Review other alternative medications to treat generalized anxiety disorder. Include risks, benefits, and potential side effects to both the patient and the developing fetus. Keep in mind, the patient is looking to breast feed for 6 months postpartum.
  • The patient agrees that it would be safest for her pregnancy and fetus to discontinue the lorazepam. How would you recommend she discontinue lorazepam? Provide education on potential side effects from tapering off the medication, including common side effects to more serious side effects and when to seek medical attention.
  • The patient would like to forgo medications at this time, given she is early in her pregnancy and is concerned about “damage” to the fetus if she were to continue medications. Provide education to the patient about the risks of untreated anxiety symptoms during pregnancy for both the patient and the fetus.

Patient 4

  • Review potential side effects for elderly on benzodiazepines providing education to both the patient and the patient’s daughter. What are the risks of continuing the benzodiazepine for this patient?
  • How would you evaluate the patient for these side effects?
  • The patient and daughter agree he will need to taper off the clonazepam given the risks of continuing this medication. How would you recommend tapering off this medication?
  • Review with the patient and daughter potential side effects of tapering off the medication. Review with them common side effects to more serious side effects and when to seek medical attention.

When working with students with disabilities, developing challenging, measurable IEP goals is essential to meet student needs and help students progress. IEP goals must be based upon the information that is contained in the PLAAFP and inclusive of individualized developmental and academic content goals. Understanding the difference between accommodations and modifications is an essential skill. Once IEP goals have been established, it is important to identify and implement appropriate accommodations and modifications to meet the unique needs of the student across educational settings. In many cases, the use of assistive technologies can be a helpful tool in applying the selected accommodations and modifications.

Review the “Charley” case study below and the PLAAFP that you wrote for Charley in the Topic 4 assignment.

Case Study: Charley

Preschool, Age 4

Charley is an energetic 4-year-old preschool student with cerebral palsy. He is in his second year of preschool. His mother experienced a difficult pregnancy with him, and she had an emergency C-section due to the umbilical cord being wrapped around his neck. Charley was diagnosed with cerebral palsy at 18 months of age. He began receiving services in home at the age of 2.

Charley typically speaks in two-word sentences. He indicates his wants and needs by pointing. He gets frustrated when he is unable to communicate quickly. He prefers to do things independently. Charley was evaluated by a speech pathologist who determined he would benefit from a speech device. He began using the speech device, both at home and school, at the beginning of the school year. He is still learning to use it consistently. Charley tries to communicate without the speech device but will use it when asked. The parents are consistent with using the speech device at home.

Charley is in a wheelchair and has limited use of his arms. He can bunny hop on the floor. He needs some assistance with feeding and is still in diapers. He receives physical and occupational therapy twice a week. Charley demonstrates frustration when he is not able to be with the other students due to these activities.

Charley loves going to school. He interacts well with all of the students and teachers. He actively participates in all activities. He tends to shut down when the content of the lesson is too difficult for him. Charley can match objects to colors and can point to 17 of the letters of the alphabet and the numbers 1-10. He requires assistance with tasks such as cutting, gluing, etc. but he is able to use larger pencils and markers for some activities.

Charley’s parents are very involved in his school. His mother volunteers in the classroom once a week, and both parents have volunteered for field trips. They are Charley’s biggest cheerleaders! They help him when he needs it, but also push him to be as independent as he can. They have expressed concern about Charley’s future in school. They would like for him to be in a general education classroom when he begins kindergarten.

In the “Individualized Education Program (IEP)” section of the “IEP Template,” write a measurable annual goal for each of the five areas on the IEP.  These goals must be based on the data you included in the PLAAFP statement you wrote for Charley in the Topic 4 assignment.

For each of the five areas on the template, include the following:

  • Standard: Specify a specific standard to which the goal aligns.,
  • Annual Goal: Specific measurable goal based on the documentation provided in the PLAAFP. Specify the baseline level of mastery and the service providers that would be involved in helping the student meet the goal.,
  • Accommodations: Accommodations related to each goal. Two of these accommodations must include the use of augmentative and alternative communication systems and/or assistive technologies. Assistive technology does not necessarily denote a high-tech process.,
  • Modifications: Modifications related to each goal.,
  • Monitoring: Explanation of how the goals will be monitored including the frequency of monitoring by whom and what tools and technology will be used to collect and store the data.,

Part 2: Explainer Video

Now that you have written the IEP, it is important to be able to explain it clearly to Charley’s parents. Record a 2-3 minute video for Charley’s parents to present the IEP goals, modifications, accommodations, and augmentative and alternative communication (AAC) that has been selected using any method of your choice. Utilize family-friendly language and engage the parents in a way that is ethical and sensitive and reflective of the tenets of the Christian worldview perspective so they feel like a valued member of the IEP team working to support Charley in attaining his goals. The video should address the following:

  • Summarize the IEP goals and the data from the PLAAFP they are based on.
  • Explain the collaboration with families and other team members that was used to develop the goals and interventions to promote positive outcomes for Charley.
  • Describe how the modifications and accommodations you have recommended support the IEP goals. Include discussion about why it is important to consider the short- and long-term effects of the use of modifications and accommodations as Charley transitions through the continuum of education and works toward post-secondary goals.
  • Describe specific education and transition plans for when Charley participates in other settings (i.e., specials, lunch, field trips). Specify the data that supports these plans and the collaboration required to accomplish the plans.
  • Explain the technology and processes that will be used for communicating information about Charley to all IEP team members in a legal and ethical manner.

Remember that video and audio quality are important and should be taken into consideration. Ensure your video is filmed with sufficient lighting and that the volume is adequate for viewers to hear. Intonation, expression, proper speech, and professional attire are expected.
