Paloma Hernandez, 26-year-old, Spanish speaking patient who presents to the clinic for the last 2 days in a row complaining of abdominal pain that is getting worse. The first visit the staff relied on her younger bilingual daughter to translate. She was treated with Omeprazole and encouraged to take OTC medication. Today she presents with the same problem. Her daughter states it is the same problem but worse today.

May 2012, Alice Randall wrote an article for The New York Times on the cultural factors that encouraged black women to maintain a weight above what is considered healthy. Randall explained—from her observations and her personal experience as a black woman—that many African American communities and cultures consider women who are overweight to be more beautiful and desirable than women at a healthier weight. As she put it, “Many black women are fat because we want to be” (Randall, 2012).

Randall’s statements sparked a great deal of controversy and debate; however, they emphasize an underlying reality in the healthcare field: different populations, cultures, and groups have diverse beliefs and practices that impact their health. Nurses and healthcare professionals should be aware of this reality and adapt their health assessment techniques and recommendations to accommodate diversity.

Mrs. Georgia D’Angelo is 76 years old. She has been married to her husband Marcus for 58 years. Marcus is 80 years old. The D’Angelo’s live in a well-kept, middle-class home. This news was devastating to the D’Angelo’s and their family and friends. Mrs. D’Angelo called a friend who is a nurse and asked who she might go to for a second opinion. A second opinion was obtained, and the second opinion reaffirmed the diagnosis of pancreatic cancer; however, the recommendation was to have chemotherapy prior to the Whipple Procedure. Mrs. D’Angelo began the chemotherapy and has suffered severe issues secondary to the chemotherapy and her quality of life is greatly impacted with a huge decrease in her functional status and activities of daily living. Social, economic, cultural, and lifestyle behaviors can have an impact on an individual’s health and the health of a community. These behaviors may pose health risks, which may be mitigated through lifestyle/behaviorally based education. The environment, housing conditions, employment factors, diet, cultural beliefs, and family/support system structure play a role in a person’s levels of risk and resulting health. Assessment, evaluation, and inclusion of these factors provide a basis for the development of an individualized plan. The health professional may use a genogram or sociogram in this process.

What is a genogram? A genogram, similar to a family tree, is used to gather detailed information about the quality of relationships and interactions between family members over generations as opposed to lineage. Gender, family relationships, emotional relationships, lifespan, and genetic predisposition to certain health conditions are components of a genogram. A genogram, for instance, may identify a pattern of martial issues perhaps rooted in anger or explain why a person has green eyes.

What is a sociogram? A sociogram helps the health professional to develop a greater understanding of these factors by seeing inter-relationships, social links between people or other entities, as well as patterns to identify vulnerable populations and the flow of information within the community.

Implementation Design

You have fine-tuned your identification of goals and objectives for both your targeted program and the evaluation you are prescribing. Your peers and supervisor have encouraged you to build your presentation for the stakeholder’s meeting by spending more time on the implementation plan.

Your goal is to prepare an additional 15–20 slides (with comprehensive speaker notes) to augment your presentation. You decide to offer a clear picture of how you intend to operationalize your evaluation.

Presentation of need, intentions, goals, and objectives
Describe the required financial and human resources.
Present your embedded evaluation strategy.
What evidence of success will you be looking for?
What problems do you need to anticipate and monitor?
Include a summary of your risk assessment plan.
Summarize contingency plans.

How will you discover the problems that require attention?
Include a description of how you intend to acquire the data supporting these.
How will data be collected?
How will the data be organized and maintained?
How will it be analyzed?
Include descriptions of the calipers shaping your ideology and the theory or theories that influence your evaluation strategies.

Your virtual team has had its second meeting and laid out the requirements for this project, which consist of the following:

Management has informed the team that the release date has moved up two months due to updates on the company’s system architecture. You feel that this is a significant event and have requested an ad hoc meeting to discuss the impacts of this change. Your team has also been notified that a seventh team member will be added from a firm in Thailand that your company recently acquired. This team member (a software tester) has a general understanding of the English language and minimal ability to read and write in English. Team Member 5 has voiced concern that meeting times have impacted his work schedule and requested that the team take into consideration the time difference. Team Member 2 has missed the last two meetings and has not responded to any emails sent from the team.
